Referral Guidelines for GP Colleagues

For comprehensive assessment of medical conditions including cognitive impairment, falls, medication reviews and social situations, please include the phrase "Comprehensive Geriatric Assessment" in the referral letter. This review will take about 60-75 minutes to complete. Patient needs to be at least 65 years old to qualify for a review.

Patients with memory/cognitive issues, please consider having a family member/carer to attend the appointment with them. A collateral history will help tremendously with the assessment and discussion of a management plan.

Referring doctors to please ensure basic blood work up has been done in the last 3 months prior to an appointment. This may include but not limited to FBE, MBA20, TFTs, B12/Folate, Vitamin D level, Iron studies, Fasting lipids including HDL, LDL & HbA1c. This will help streamline their initial review so that a comprehensive medical plan can be obtained on the first consultation. CT Brain/MRI Brain is optional.

Please kindly include an updated medication list and problem list/condition that needs a specific review in the referral letter.
